FORM B – DEFFERAL
Use this form to request permission for persons 11 years old to defer writing the examination and remain in primary school for one additional year.

FORM C – Special Request
Use this form to request that a student be exempted from writing the examination. This form can also be used to request accommodation for students with special needs eg. vision impairment, dyslexia, diabetes, muscular dystrophy, etc.
